Getting a Referral

Patients must obtain a physician referral before being admitted to Walton Rehabilitation Hospital or receiving treatment at our Outpatient Center. Contact your doctor to determine if Walton is right for you. A referral is not required to receive treatment at the Walton Pain & Headache Center.

Once you have your referral, please contact us to get started. We will need the patient's address, date of birth, next of kin and insurance information. Our rehabilitation services are covered by most insurance policies. For a complete list, click here. If appropriate, financial assistance plans can also be discussed.
