| Description: |
100% of preventative services are covered, 80% of basic services and 50% of major services with a $50 deductible per insured. Each person covered under the plan receives two teeth cleanings annually with no additional out of pocket expense. Employees have no restrictions on dental providers. Premiums are shared with employees and deducted pre-tax through payroll deduction. Coverage is available to full-time employees and eligible dependents 31 days after hire. |
