Hospital inpatient & outpatient services
If you have experienced a stroke, brain injury, spinal cord injury or other traumatic injury, or are recovering after orthopedic surgery, Walton Rehabilitation Hospital or Walton Outpatient Therapy Center may be right for you. Your doctor must provide a referral in order for you to receive treatment at Walton.

Upon your referral, contact us to begin the admission process. Please be prepared to provide:
- Patient's address
- Date of birth
- Next of kin
- Insurance information
Our rehabilitation services are covered by most insurance policies. For a complete list, click here. If appropriate, financial assistance plans can also be discussed.

Arriving at Walton
When you arrive at Walton, our admissions representative will greet you and your family, explain your rights as a patient and assist you in signing the consent form and other forms needed. If you are unable to sign, your family member may sign on your behalf.
Make sure you bring the following:
- Insurance cards or proof of Medicare or Medicaid, with policy holder's name and employer's name
- Living will, power of attorney or advanced directive (if applicable). The admissions representative will ensure a copy is placed in your medical record.
