Hospital inpatient & outpatient services
Patients must obtain a physician referral before being admitted to Walton Rehabilitation Hospital or receiving treatment at our Outpatient Center. Contact your doctor to determine if Walton is right for you.
Once you have your referral, please contact us to get started. We will need the patient's address, date of birth, next of kin and insurance information. Our rehabilitation services are covered by most insurance policies. For a complete list, click here. If appropriate, financial assistance plans can also be discussed.

What happens when I am admitted?
On the day you arrive at Walton, you will be met by an admissions representative. The admissions representative will explain your rights as a patient and assist you in signing the consent form and other forms needed. If you are unable to sign, a family member should be with you to sign the consent form. If you have a living will, power of attorney or advanced directive, please bring a copy. The admissions representative will ensure that a copy is placed in your medical record. Please bring your insurance cards or proof of Medicare or Medicaid. We may need additional information such as the policy holder's name and employer's name.
